This complete comprehensive SOAP note must include a full subject,
objective history and physical exam as well as an assessment and plan of care

identified health problems (current and/or past diagnosis) in order that the
student may synthesize knowledge from family and primary care theory.
The patient’s chief complaint is:
24 year old female patient who is in college complains of burning foul-smelling urine. Patient
states that she has some lower abdominal discomfort. And she often uses the restroom
frequently and has little dribbling urine. This has been going on for the past week. She also
starts to notice that shes having weird vaginal discharge and that shes starting to
smell fishy despite always washing and taking a proper shower.
