My change proposal: I am a float pool nurse who works in the emergency room, orthopedics, med-surg,
cardiac care units, and progressive care units. Patients are frequently discharged without adequate teaching or
with the appropriate resources to successfully care for themselves outside of the hospital. My proposal is to
create a position( discharge Navigator) that would only focus on the discharge process for patients. This would
involve working with a team, creating a budget, and having frequent meetings to determine if there are any
barriers and if certain goals are being met.
