According to his teacher, Frankie is a 9-year-old boy who is constantly “into everything.” He keeps the class in an uproar and has been suspended from school three times this year, most recently for swinging from a light fixture. He wanders around the classroom, talking to all the children. So, not only does Frankie not accomplish his work, the other students cannot complete their work. Frankie’s mother reports that Frankie’s behavior has been difficult since he was a toddler when he became unbearably restless and demanding. He slept very little and “got into everything.” When he was 4 years old, he was rejected by a preschool because of his difficult behavior. He has few friends because he is unable to participate in fair play. He cannot watch TV or participate in games that require quiet concentration. His activities at home essentially include riding his bike or playing outdoors with his dog. His room stays messy, and he is destructive of his possessions. The psychiatrist admits Frankie to the child psychiatric unit with a diagnosis of ADHD. He orders the initiation of therapy with methylphenidate (Ritalin).
