Create a workflow step by step telling the staff medical assistant and providers step by step
For me to present in all staff meetings
The plan is to create a flyer for the patients telling them to use the FIT testing
Process of workflow
1. Chart Review (ahead of time) can be done the morning of the clinic look for care manager rooming form pre-visit prep… colon screen tile if its red check if patient is due or not and where to look just check care manager (pics at the end)
2. Get a Fit Kit envelope ensure test put it in the room with patient handout for provider to discuss with the pt and let provider know pt is due
3. Provider discusses with patient the testing and orders the test (under cancer custom list)
4. Patient takes Kit home to complete
5. EMR team pulls list of orders placed
6. Quality team enters patients name synced with the orders on outreach log (Rosabelle) I won’t have access
7. Quality team (me) calls pts at 2 weeks after appt Confirms test completed does short survey or answers questions pt survey about the process
8. Quality team will review results in chart (2 weeks later … f/u results pulled into the log) (Rosabelle)
