concern:

work
6.    For each
area of concern, specifically describe the behavior, skill and/or attitude of concern.
(While subjective, it’s important to clearly describe
specific examples to
illustrate areas of concern.)
7.    For each
area of concern, list a minimum of
one constructive and specific objective (think SMART goals) the counselor/trainee must meet in order to succeed. Provide a specific time frame for each objective.
8.    State the
end date for the Individual Remediation Plan.
Be sure to include a statement similar to the following: Obtaining the total hours required by the
counseling program/licensing board is a minimum requirement, this plan may
require additional clinical
hours to assure
the trainee/candidate meets
minimum standards for the program/licensing board.
